Where applicable, I have received proper and informed permission from all individuals whose likeness is included in the Image and/or proper and informed permission from any other third party who owns or can claim any rights in the Image. I certify that the Image does not contain inappropriate images including but not limited to drugs, nudity or any other explicit content. I certify that any individual appearing in the Image was at least 21 years of age when the Image was created.

I, hereby grant to JFW the perpetual rights to display, reproduce, edit, publish and otherwise use, in any form anywhere in the world, forever after, the Image, as made in connection with JFW’s promotional campaigns or materials, including without limitation use on JFW’s websites, social media pages, or other promotional materials, with or without all or any part of my name, signature, endorsement and biographical data, concerning me, for any and all purposes, in any and all present and future media.
